Swati is an Adult Nurse Practitioner with a long-standing passion for caring for people with neurological conditions, especially dementia and Alzheimer’s disease.

She brings nearly 20 years of nursing experience to her role, along with a calm, compassionate approach that helps patients and families feel supported every step of the way. She holds a Master of Science in Nursing (Adult Nurse Practitioner) from the University of North Carolina at Charlotte and a Bachelor of Science in Nursing from the University of North Carolina at Chapel Hill.

Her career began at Atrium Health, where she worked as a registered nurse in the Medical Intensive Care Unit and on the progressive stroke unit. Those years gave her a strong foundation in critical care and a deep respect for what patients and families go through during serious illness. After earning her master’s degree from UNC Charlotte, she became a nurse practitioner and continued working in stroke and neurology—leading inpatient and outpatient programs, improving care processes, and helping patients recover and prevent future strokes. Over time, her focus shifted toward dementia and movement disorders. At Memory & Movement Charlotte, she helped patients and caregivers manage conditions like Alzheimer’s and Parkinson’s disease. She also introduced new technologies and care workflows to make the patient experience smoother and more connected.

Today, Swati is part of the care team at Isaac Health, where she provides dementia and Alzheimer’s care through telehealth. She also helps lead data and program efforts for the GUIDE (Guiding an Improved Dementia Experience) model, a national initiative supported by Medicare to improve dementia care across the U.S. In addition, she contributes to the ALZ NET registry through the Alzheimer’s Association, helping to advance research and understanding of disease-modifying treatments.
