    Complete your first appointment

    You will meet with a neuropsychologist and a specialist doctor for a comprehensive brain health assessment. They will spend dedicated time getting to know you, listening to your concerns, and performing a detailed neuropsychological and neurological assessment.

    Finish labs and imaging tests

    Following your first appointment, we may request lab work at an imaging center near you. This is a vital step in ensuring your care team has the clearest possible picture of your health.

  3. Isaac Health Patients

    Meet your Isaac Health care team

    A few weeks after completing your labs, you’ll have a follow-up appointment with your care team to discuss your results and get a clear understanding of your brain health.

  4. Get a personalized treatment plan

    During your care planning session, your care team will share your treatment plan, which may include cognitive rehabilitation therapy, speech therapy, and medication management.

  5. Receive ongoing care & monitoring

    If you are eligible for the CMS GUIDE program, you will also have access to a 24/7 support line, care navigation, caregiver support, and respite care covered by Medicare. If you are not eligible for the GUIDE program, you will have the option to enroll in our Care Navigation program, which pairs you with a dedicated care navigator to help you manage appointments, access community resources, and much more.

Care Navigators at Isaac Health help you coordinate appointments with your care team, access local and online resources for support, and check-in with you periodically to make sure that you’re feeling supported.
