2026 outcomes study on inpatient utilization in Medicare populations Isaac Health's care model was shown to meaningfully reduce acute care utilization
In this observational study of Medicare beneficiaries, enrollment in the Isaac Health program was associated with reduced inpatient hospital admissions over a 6-month follow-up period. These findings suggest that a specialist-led, virtual brain health model can reduce acute care utilization in a medically complex population and that enhancements to the Isaac Health model may be particularly important in driving this effect. Notably, the observed reduction in inpatient admissions appears greater than what has typically been reported for earlier dementia care management models.

The Centers for Medicare & Medicaid Services (CMS) Guiding an Improved Dementia Experience (GUIDE) Model was designed to test whether a comprehensive dementia care model could improve outcomes while reducing total cost of care. Since July 2024, Isaac Health has participated in the GUIDE Model across all 50 states. Early performance results verified by CMS demonstrate strong outcomes, including meaningful reductions in total cost of care and lower-than-expected admissions to long-term nursing homes.

In this study independently verified by the Validation Institute, emergency room visits and hospital days decreased significantly for Isaac Health participants in comparison with a matched comparison group. This reduction in unnecessary utilization is associated with a notable decrease in PMPM costs for Isaac Health members.
