The Guiding an Improved Dementia Experience (GUIDE) program is a Medicare program that supports Medicare beneficiaries living with dementia, as well as their families. As a member of our GUIDE program, you and your loved ones will have access to a dedicated medical team, caregiver support, and respite care, among other supports and services fully covered by Medicare.
Isaac Health is always there for you and your family. As a member of our GUIDE Program, you will have access to dementia-trained specialists and support staff through our 24/7 support line.
Your care navigator will be your personal guide through the whole journey and will check in with you or your loved one regularly. They can also make referrals to community support organizations.
A dementia care specialist can visit your home to review safety, identify potential risks, and help create a secure environment that supports independence and prevents falls.
Our specialist doctors and nurse practitioners will perform a detailed assessment upon enrollment in GUIDE and at least once a year. Your medical team will create a personalized treatment plan for you or your loved one.
We host caregiver training sessions for our GUIDE members. Our specialized caregiver coaches also provide 1:1 sessions to go over what dementia is and what it may mean for your family. Additionally, we run support groups and group activities for members and families.
Everyone deserves to take a break. Respite care allows caregivers to take a temporary break from caregiving, while a trained home care aide comes to your home to look after your loved one. Through our GUIDE Program, you will have 20 annual sessions for 4-hour in-home respite care.
Our care team will help you determine if you or your loved one qualify for the Medicare GUIDE Model and guide you through the enrollment process — no referral needed.
Your Isaac Health care team will create a customized dementia care plan based on your needs and goals. You’ll also be paired with a dedicated Care Navigator who helps coordinate your services and supports you every step of the way.
If you have a caregiver, they’ll have access to training sessions, one-on-one coaching, and support groups to build skills and find emotional support.
Receive regular check-ins, memory assessments, and coordinated care from a team specializing in dementia and brain health. We’re here to help you stay healthy and supported as needs change over time.
To be eligible for the GUIDE Program, you must be enrolled in Medicare Parts A and B with Medicare as your primary insurance. You are not eligible for GUIDE if 1) your primary insurance is Medicare Advantage or another Medicare health plan (including Special Needs Plans and the Program of All Inclusive Care for the Elderly), 2) you are enrolled in Medicare hospice benefit, 3) if you are a long-term nursing home resident (defined as a nursing home stay of 4 months or longer and not covered under the Medicare skilled nursing facility benefit), or 4) you are enrolled in a PACE program.

We work with most major insurance plans for non-GUIDE services, which include assessment, treatment, and care management for dementia.
If you are eligible for GUIDE, you will be fully covered by Medicare for all services you receive through the program.
