What is the CMS GUIDE Program?

The Guiding an Improved Dementia Experience (GUIDE) program is a Medicare program that supports Medicare beneficiaries living with dementia, as well as their families. As a member of our GUIDE program, you and your loved ones will have access to a dedicated medical team, caregiver support, and respite care, among other supports and services fully covered by Medicare.

    Caregiver support

We host caregiver training sessions for our GUIDE members. Our specialized caregiver coaches also provide 1:1 sessions to go over what dementia is and what it may mean for your family. Additionally, we run support groups and group activities for members and families.

  • Nurse with patient

    Respite care

Everyone deserves to take a break. Respite care allows caregivers to take a temporary break from caregiving, while a trained home care aide comes to your home to look after your loved one. Through our GUIDE Program, you will have 20 annual sessions for 4-hour in-home respite care.

Testimonials

Since enrolling in the GUIDE program through Isaac Health we have received exceptional assistance both for my wife with her Alzheimer's and myself as her caretaker. I can honestly say I don't know where she and I would be today if I had not enrolled with them and received the physical and psychological help they have given us. This is a godsend of a program. If it is available to you, reach out and take advantage of what it can do for you.
