Kathryn Lesane, LCSW is an accomplished licensed clinical social worker with more than a decade of experience in behavioral health therapy, clinical case management, and healthcare leadership.
Her clinical work focuses on behavioral health and emotional wellbeing, providing therapy for individuals and families coping with anxiety, depression, grief, chronic illness, and cognitive decline. Her therapeutic style is trauma-informed and strengths-based, combining supportive counseling, cognitive-behavioral interventions, and solution-focused techniques to help clients build resilience and achieve meaningful change. She is deeply committed to helping patients and caregivers navigate emotional transitions with understanding and empowerment.
Equally grounded in care coordination and program leadership, Lesane has extensive experience developing and managing social work and case management programs across diverse healthcare settings. I was drawn to Isaac Health because of its commitment to whole-person, compassionate, and evidence-based dementia care. The organization’s integrated model (where behavioral health, neurology, and care coordination work together) aligns closely with how I believe care should be delivered: comprehensive, accessible, and deeply human.
Dementia care has always been a passion of mine because it blends both the clinical and emotional aspects of social work. Supporting individuals and families through memory loss is not only about symptom management, but also about preserving identity and dignity. Working in this field allows me to combine therapeutic support, education, and advocacy to improve quality of life for both patients and families.
