As an Advanced Practice Provider for over a decade, she has a wealth of experience spanning hospital medicine, inpatient palliative care, community based geriatrics, and outpatient geriatrics and palliative care. She is passionate about helping families navigate the community health system to leverage care that supports aging well.

Meshack-Hart utilizes a collaborative approach which incorporates care partners and loved ones into patient-lead conversations and care planning. She strongly believes that supporting care partners in the community is key to the success and quality of life of patients with cognitive impairment. She is also passionate about Lifestyle Medicine principles and how they can support healthy aging.

Why I joined Isaac Health

I have valued aging members of society since I was very young, starting with my own grandparents and great aunts and uncles, and have always been drawn to this population. Isaac Health represents a comprehensive health model that everyone deserves, and does so in a way that improves access to care for the most vulnerable and those in rural communities.
