White Paper The Economic and Clinical Case for Specialized Dementia Care

Dementia drives some of the highest hospitalization rates in Medicare and Medicaid, yet most patients wait months to see a specialist. A new outcomes report from Isaac Health shows that virtual, specialist-led dementia care can change that. In a matched cohort analysis, enrollment in the Isaac Health program was associated with a significant reduction in hospital admissions over just six months. The observed reduction in inpatient admissions appears greater than what has typically been reported for earlier dementia care management models. Download the report to learn:

  • How Isaac Health’s model builds on leading dementia care management programs like UCLA Health’s ADC and UCSF’s Care Ecosystem
  • Which clinical and care management gaps drive avoidable hospitalizations in dementia patients
  • Why specialist-led virtual care may be the key to reducing acute utilization in high-risk Medicare populations
